Scenario

You have already learned about evidence-based practice and quality improvement initiatives in previous courses. You will use this information to guide your assessments, while also implementing new concepts introduced in this course. For this assessment, you will develop a concept map and provide supporting evidence and explanations. You may use the case studies presented in the Vila Health: Concept Maps as Diagnostic Tools media, a case study from the literature or your practice that is relevant to the list of conditions below, or another relevant case study you have developed. This case study will provide you with the context for creating your concept map. You may also use the practice context from the case study or extrapolate the case study information and data into your own practice setting. Think carefully when you are selecting the case study for this assessment, as you may choose to build upon it for the second assessment as well.Concept Map with Brief Narrative for Diabetes Essay.

Instructions

Develop a concept map and a short narrative that supports and further explains how the concept map is constructed. You may choose to use the Concept Map Template (in the Resources) as a starting point for your concept map, but are not required to do so. The bullet points below correspond to the grading criteria in the scoring guide. Be sure that your evidence-based plan addresses all of them. You may also want to read the Concept Map scoring guide and the Guiding Questions: Concept Map document to better understand how each grading criterion will be assessed.Concept Map with Brief Narrative for Diabetes Essay.

Part 1: Concept Map (example and template uploaded below)

  • Develop an evidence-based concept map that illustrates a plan for achieving high-quality outcomes for Diabetes type 2  as related aspects. Identify assumptions on which plan is based.

Part 2: Additional Evidence (Narrative) (example uploaded below)

  • Justify the value and relevance of the evidence you used as the basis for your concept map. Impartially considers conflicting data and other perspectives.Concept Map with Brief Narrative for Diabetes Essay.
  • Analyze how interprofessional strategies applied to the concept map can lead to achievement of desired outcomes. Identifies knowledge gaps, missing information, and unanswered questions.
  • Construct concept map and linkage to additional evidence in a way that facilitates understanding of key information and links. Identifies specific strategies or approaches used to ensure clear communication.
  • Integrate relevant sources to support assertions, correctly formatting citations and references using current APA style.

Additional Requirements

  • Length of submission: Your concept map should be on a single page, if at all possible. You can submit the concept map as a separate file, if you need to. Your additional evidence narrative should be 2–3 double-spaced, typed pages. Your narrative should be succinct yet substantive.Concept Map with Brief Narrative for Diabetes Essay.
  • Number of references: Cite a minimum of 3–5 sources of scholarly or professional evidence that supports your concept map, decisions made regarding care, and interprofessional strategies. Resources should be no more than five years old.
  • APA formatting:
    • For the concept map portion of this assessment: Resources and citations are formatted according to current APA style. Please include references both in-text and in the reference page that follows your narrative.

Introduction

This paper offers an evidence-based concept map that illustrates a nursing care plan for a patient experiencing fatigue, weight loss, suboptimal diabetes control and foot pain. The concept map includes the patient information, differential nursing diagnoses, appropriate interventions, the rationale for each intervention, and desired outcomes. The presented narrative justifies the evidence applied in the concept map and offers additional evidence to support the nursing care interventions suggested.Concept Map with Brief Narrative for Diabetes Essay.

Additional Evidence

John Doe is a 69-year-old male retiree who is experiencing fatigue, recent weight loss, suboptimal diabetes control, and foot pain. He has a history of type 2 diabetes for which Diabeta was prescribed, but he stopped taking the medication citing unfavorable side effects that included mild agitation, sweating, and dizziness. The differential nursing diagnoses presented for the patient were based on the medical data and history as well as presenting complaints and symptoms.Concept Map with Brief Narrative for Diabetes Essay. The evidence used to make the three differential nursing diagnoses is based on a combination of subjective and objective data obtained after conducting medical investigations. A review of Mr. Doe’s history reveals that he was diagnosed with type 2 diabetes and had symptoms of hyperglycemia before the diagnosis. The evidence suggests that the patient current distress is related to type 2 diabetes, and particularly metabolic syndrome, hyperthyroidism, and/or insulin resistance and hyperglycemia. The metabolic syndrome diagnosis is based on the observed abdominal obesity, high blood pressure and tryglycerides levels, and low level of HDL cholesterol. The hypothyroidism diagnosis is based on fatigue, weight loss, excessive sweating, and muscle weakness reported by the patient, and a confirmed thyroid function test. The insulin resistance and hyperglycemia diagnosis is based on fatigue, slow healing wounds, and foot paresthesia reported by the client, and high A1C and fasting glucose levels (Dunning, 2014; Thomas, 2013).Concept Map with Brief Narrative for Diabetes Essay.

Nursing Intervention

Although the three differential nursing diagnosis have similar symptoms, nursing management is different. In this case, the nursing interventions include the actual actions and treatments performed by nurses to help the patient to reach the health goals. They involve the nurse using critical thinking skills, experience, and knowledge to determine the interventions that will help the most. Three types of interventions have been applied.Concept Map with Brief Narrative for Diabetes Essay. Firstly, the metabolic syndrome intervention applies an interdependent intervention that requires the participation of a dietician and nutritionist to help plan an appropriate diet plan. Secondly, the hypothyroidism intervention applies a dependent intervention since a physician order is required to conduct a thyroid function test. Finally, the insulin resistance and hyperglycemia applies an independent intervention that involves the nurse initiating the treatment by prescribing medication. The nursing interventions reveal that nurses’ scope of practice have implications for their participation in treatment efforts (Rosdahl & Kowalski, 2008).Concept Map with Brief Narrative for Diabetes Essay.

Value and Relevance of Concept Mapping

Concept mapping is a good way for connecting information. As the differential diagnosis are explored, the concept map perceives them as new knowledge before integrating and connecting them to existing knowledge so that nurses find it easier to comprehend and recall the information. In fact, the concept map has been used to build and organize nursing ideas, layer details, and find relationships and connections that could have easily been ignored (Schuster, 2016). Besides that, the map helps to visualize, sequence, hypothesize, evaluate, compare and contrast, brainstorm, and analyze information relevant to the provision of nursing care (Schmehl, 2014).Concept Map with Brief Narrative for Diabetes Essay.

Conclusion

Type 2 diabetes is a medical condition that is linked to other comorbidities. Identifying and targeting individual comorbidities can help in type 2 diabetes management through symptom reduction. Concept mapping helps in organizing the information on the comorbidities by summarizing the related information. As a result, a concept map is an important tool for determining the appropriate nursing interventions to achieve the best possible care outcomes.
