This paper discusses the PICOT question that the author will explore for the capstone change project addressing substance use disorder. The author will also discuss the nursing intervention, clinical problem, patient outcomes, patient care, and healthcare agency related to the PICOT question that will guide the implementation of the change project to influence patient outcomes.
PICOT question: Among patients with substance use disorder (P), does the use of CBT (cognitive behavioral therapy) (I) compared to no behavioral therapy (C) result in improved quality of life (O) within 12 weeks? (T)
Nursing Intervention
The nursing intervention to address the PICOT problem is utilizing cognitive behavioral therapy. The recent use of cognitive behavior therapy as a form of psychotherapy has been significant in preventing addiction, relapse, and improvement of thought connectivity. It also enables the patient to realize their actions, feelings, and the full awareness of themselves, promoting recovery from addiction and substance abuse(Vujanovic et al. (2016). CBT delivered as group therapy has better outcomes than individual therapy in managing patients with a severe addiction to drug use
Clinical Problem and Patient Outcome
Diagnostic and Statistical Manual of Mental Disorders, 5th Edition defines substance abuse as a mental illness that results from abusing ten different drugs. These drugs include; alcohol, caffeine, cannabis, inhalants, opioids, sedatives, hypnotics, anxiolytics, hallucinogens (phencyclidine or arylcyclohexylamines, and LSD), stimulants (amphetamine-type substances, cocaine, and other stimulants), tobacco, and other or unknown substances. The mechanism of action of these drugs uses different pathways in the brain. Commonly, the neurotransmitters affected include; epinephrine, norepinephrine, dopamine, and serotonin. Prolonged use of these drugs promotes tolerance, which progresses into dependence after that addiction. Substance abuse encourages the development of a psychoactive maladaptive behavior generated by the consumption of addictive drugs (Magill et al., 2019). Cognitive-behavioral therapy is the most commonly used therapy among patients admitted for alcohol addiction and other highly addictive drugs. The therapy is broad, directive, and structured with several techniques that aim to reduce and remove maladaptive ways of beliefs, thoughts, attitudes, and cognition. These elements as translated into a behavior that is considered the standard for psychological interventions. According to Martín-Pérez et al. (2019), cognitive behavioral therapy is the most effective treatment plan for patients with alcohol dependence and substance use addiction. This therapy provides for tertiary preventions of drug consumption and a reduction in the frequency of abuse. Therefore, patients initiated on the therapy have better outcomes compared to those on other behavioral therapies.
Nursing Intervention
The nursing intervention of cognitive behavioral therapy is important for patients with substance use disorder to improve their quality of life. It enables reduction of maladaptive behaviors and enhance patient capacity to overcome addition. The use of CBT has shown efficacy to improve patient’s adjustment to new patterns of life that are significant. Patients who cannot coordinate life activities due to drug addiction can quickly realize their potential and walk the part of improving their lives.Chiang et al. (2017) describe that patients with analcohol addiction and substance use disorder can improve the control of their emotions, feelings, and thoughts and reduce anxiety. Patients with mental health disorders can experience difficulty controlling symptoms, especially when confronted by an agent that aggravate the illness. However, several studies have proven an evidence-based practice by using both CBT simultaneously with pharmacological treatment of severe anxiety disorders. Pharmacological therapies suppress anxiety symptoms while CBT enhances the realization of how to overcome the anxiety.
Health Care Agency
The author purposes to implement this project in a large metropolitan hospital that provides psychiatric inpatient and outpatient care to a diverse population. In the recent past, substance use has increased among the young population and majority are developing addiction to these drugs, especially in urban centers.
Nursing Practice
Although the project will incorporate multiple stakeholders, nurses will play a leadership role in ensuring the success of this project. The primary role will be engaging patients on various activities available in this therapy, supporting patients and performing evaluations on their progress. Nurses will also facilitate the implementation of this therapy among patients who have been enrolled.
