PEER RESPONSE: According to Tan- Fu Hsueh and Dorcy (2016), there are numerous areas for improvement when it comes to the transition of a patient from inpatient to outpatient treatment. This can especially be said in regards to cancer patients due to the patient typically having multiple other comorbidities that need to be addressed and properly resourced and supported when the patient leaves the hospital.Bio Statistics Research Nursing Essay.  The authors conducted a pilot study at the University of Washington Medical Center, which included an Advanced Practice Nurse Care Coordinator (APNCC) that helped identify those errors or transition gaps to help better understand where in the process there can be improvement. “The aim of the pilot study was to reduce length of stay (LOS), lower read- mission rates, and decrease delays and misses in treatment during transitions from inpatient to outpatient care” (Tan- Fu Hsueh, DNP, RN-BC & Dorcy, RN, PhD, 2016, p. 240). There were numerous aspects to the care in this facility that included “inadequate care planning and coordination, missed laboratory work, unprepared transfusions, missed or delayed referrals and follow-ups, ancillary care items not ready for dis- charge, causing delays, growth factor, anticoagulation, and antibiotics arrangement per insurance coverage eligibility, discharge medications dispensation barriers, inadequate central line post-discharge care, difficult to progress care because of behavior or social issues, serious event or care delay reviews” (Tan- Fu Hsueh, DNP, RN-BC & Dorcy, RN, PhD, 2016, p. 241). With these topics in mind, there was major improvement and numerous goals were reached. Patient’s length of stay was reduced, there was less anxiety on the patient’s part in regards to discharge, an increase in medication compliance, a decrease in the rate of central line-associated bloodstream infections (CLABSI) and overall better communication between staff and patients. This in turn made for a better experience for everyone involved in each patient’s care and helped with the transition to outpatient services. Overall, the measurements that were chosen by the authors was deemed necessary and very appropriate for the study. This allowed many transitional gaps to be addressed and a favorable outcome was acquired. Putting these specific measurements in place would benefit patients across the nation and allow for a better experience for staff and patients if implemented. I agree with the care aspects identified within the posts as those explored in the article. In addition, the presented findings relate with the pilot study findings.
