Analysis of Study Related to Advanced Nursing Practice Essay

ORIGINAL QUESTION FOR DISSCUSION Conduct a literature search and find a peer-reviewed, single PRIMARY research article in your advanced practice area of interest. This means that the article should present an actual single research study conducted by the authors of the article —no literature reviews or articles ABOUT research conducted by others. Be sure that the research is a topic relating to advanced practice nursing. Post a 2-3 paragraph summary of the research including the research method (qualitative, quantitative, intervention, mixed method, etc) and types of measurement used to gather the data. Decide whether the measurements chosen were effective. Attach a PDF copy of the article to your post. Contact a librarian for help in completing a literature search if you need assistance. THIS IS THE PEER RESPONSE TO DISSCUSION: The primary research article I chose was Outcomes of an Advanced Practice Nurse-Led Continuing Care Program in People with Heart Failure. The study is a quasi-experimental research study with a purpose to compare the outcomes of an Advanced Practice Nurse-Led Continuing care program by two master’s degree prepared advanced practice nurses in people with heart failure (control group) with those in a comparison group receiving standard care at a university hospital in Thailand. Participants were selected for the study based on purposive sampling, meaning that the researcher consciously selects certain participants, elements, events or incidents to include in the study (Gray et al., 2017). In this study, criteria included being 45 years and older, diagnosed with heart failure from a myocardial infarction with an ejection fraction less than 40%, alert and oriented, reachable by telephone after discharge, and able to speak and understand the Thai language (Jittawadee et al., 2019). Data were obtained during the study through the use of four questionnaires. Demographic data was analyzed using descriptive statistics. Chi-square, Fisher’s exact, and t-test were used to compare differences in both groups. Outcomes measured included body weight changes, functional status on completing activities of daily living, any complications (renal failure, cardiac arrhythmia, etc.), emergency room visits, time interval between discharge and the first readmission, readmission rate, length of stay, and cost of care which are all objective, direct measurements analyzed using regression analysis with the Stata statistical software (Jittawadee et al., 2019). In addition, indirect measurements such as quality of life and satisfaction with nursing care were addressed on two other questionnaires, using multilevel analysis techniques. The measurements used to gather and analyze data were partially effective in this research study and yielded statistically significant differences in patients overall satisfaction with their care, length of stay, cost of care, quality of life, and ability to perform activities of daily living in patients who received an advanced practice nurse-led continuing care program compared with those receiving standard care. However, no statistically significant differences were identified evaluating the effectiveness of the intervention to body weight changes, and complications including emergency room visits and readmission rates between the two groups in this study (Jittawadee et al., 2019). The choice of article for the review was befitting as the article titled ‘Outcomes of an Advanced Practice Nurse-Led Continuing Care Program in People with Heart Failure’ relates very well to advanced practice nursing. In the analysis, it is noteworthy that the research methodology used and the statistical analysis of the data collected have both been discussed at considerable length. It is also noteworthy that the sampling technique that was used by the researchers was purposive sampling. The significance of this is that sampling that is not random jeopardizes the validity of the findings and hence puts doubt on the generalizability and applicability of the research findings because of the inherent bias (Palinkas et al., 2015). The sample characteristics are well discussed as including being 45 years and above and having been diagnosed with a heart failure resulting from myocardial infarction (MI). The study type is given as quasi-experimental. However, this is just left at that with no explanation as to why this study type was chosen and not others. In the methodology, it is observed that four questionnaires were used to collect data. No reason is given as to why one questionnaire would not have been enough. Four instruments appear to be too many and prone to presenting confusion during data analysis. Even though the sampling method was purposive and not random, the discussion on statistical data analysis reveals that strong nonparametric tests such as Chi-square test, t-tests, and regression analysis were used. The benefits of these analysis techniques however seem to have been reduced by the sampling technique which was not random. This has an impact on significance.

It is mentioned that part of the results were statistically significant, meaning that the p-value was 0.05 (there is only a 5% chance that the results are due to chance). However, because of the weak sampling technique used, the applicability or generalizability of the results of this study is questionable. This means that even though the results are statistically significant, they may not be clinically significant as to influence a change in practice (Ranganathan et al., 2015; Mariani & Pêgo-Fernandes, 2014).  Analysis of Study Related to Advanced Nursing Practice Essay.
