This paper will describe a proposal for a project using mHealth with a smartphone application for patients who present at the emergency room of Bergen New Bridge Medical Center for psychiatric evaluations and treatment, including those discharged from the emergency department and inpatient psychiatric care. Many patients have dual diagnoses’. There are high incidences of recidivism. Often, patients have limited support due to their mental illness, severe chemical dependency, lack of employment, and homelessness. Frequently patients are discharged to shelters with follow-up plans to return to the outpatient clinic at the hospital or to an outpatient provider in the community. Patients often return to the emergency department in crisis due to medication non-compliance and relapse on substances. Through a study done using mHealth, they found through qualitative research that mHealth had several outcomes based on where the patient was in their recovery process (Geneva et al., 2019). The proposal’s purpose suggested to Bergen New Bridge Medical Center is to provide additional support to remain medication compliant and provide interventions via mHealth smartphone such as self-management tools. Furthermore, the purpose is to achieve enhanced recovery and diminished recidivism to the hospital emergency room leading to an inpatient admission level of care. Stakeholders impacted by the project
This project’s stakeholders will be informed of the benefits of mHealth smartphone use to support patients discharging from the emergency department and inpatient mental health. The stakeholders are the patients, the psychiatric nurses and social workers, the psychiatric nurse practitioners, psychiatrists, the program directors of the emergency department, and inpatient units. The chief operating officer of the continuum of care department, quality management department and the hospital are stakeholders due to their goal of having safe, effective care and discharge plans provided to the patients present and treated at the hospital.
Explanation of patient outcomes or efficiencies for this project
The stakeholders received information on how the program will help patients comply with medications and follow-up appointments through reminder texts, weekly contact with a clinician, educational information access, and encourage coping skills. The stakeholders get informed of eHealth’s application to improve
patient outcomes (Ng, 2018). They will receive education on some successes discovered through studies such as one done where short message service is directed to a population of homeless individuals in Virginia City with reminders of appointments and medication refills (2018). Of the 89% who owned a cell phone, 77% were interested in receiving appointment reminders and 66% in medication information and refills (2018). Components identified

Furthermore, the low cost of utilizing mHealth as a tool to achieve improved outcomes for patients established through data compiled to use with the program over three, six, and twelve-month periods is proposed (2018). The program focused on patients with a history of non- compliance with medications and aftercare (2018). The use of artificial intelligence through mHealth smartphone application has shown through a study using a mHealth smartphone application called Focus benefits of support at the time needed as a coping skill (Geneva et al., 2019). Participants acknowledged while engaged in FOCUS mHealth strategies, they gained insight, a feeling of being connected and supported by others, and an improved outlook in general (2018).
What Technologies are required to implement this project, and why?
Research is needed to assess if FOCUS, an intervention using a smartphone, is obtained for a charge (Geneva et al., 2019). If there is a charge, would the cost be covered by the patients’ insurance? Do the patients who meet the program’s criteria have a smartphone, and if they do not, would there be a plan available to obtain a phone for the patient? How would the interface connect to the hospital information system? Would additional personal be needed to operate the service of this program? What would the cost of hiring a nurse, and how would it be covered? Would the nurse need to be an additional informatics nurse trained in the use of this technology? Nurses who call patients weekly for check-in need training (Williams et al., 2018). Would the psychiatric nurses in the emergency departments handle patient calls, or would another nurse be needed to facilitate the program? For the program to be successful, another psychiatric nurse would need to be hired (2018). The expectation is success based on the implementation of the program at other facilities and the documented success. The newly hired psychiatric nurse will need to be trained in evidence-based mental health resources, the FOCUS program, and included in emergency room team meetings to become familiar with the psychiatric community served (2018). These questions would be researched by the informatics nurse and reported back to leadership for approval. The information needed would need to be reviewed for the feasibility and identification of the benefits of providing the services to improve patient outcomes. The goal of hospitals, clinics, and other healthcare providers is to treat and heal to achieve the best possible outcomes, a thorough examination of the technology and the personnel to succeed. The nurse informaticist (NI) role for this project will be the Project Manager due to their dual expert knowledge of healthcare, in this case, psychiatric nursing care and information technology (McGonigle, 2017). The project team would include the Psychiatric Nurse Practitioner, Psychiatrist, policy and quality departments, and the Information Technology personnel. The nurse informaticist will conduct design, analyze the system, integrity management, oversee compliance, consult, and coordinate (Sipes, 2016). The NI will also do system development, information management, education, policy development, and quality oversight with the quality department (Geneva et al., 2019). The NI assists in evaluating, research, maintain security, and safety of the program (2019). The NI role is significant to this project to connect nursing science with the integration of analytical and information science for the benefit of the patients’ best outcomes (2019). In conclusion, the mHealth smartphone for psychiatric patients discharged from the emergency room and inpatient hospitalization project presented is meant to improve patient outcomes. The improvements are measured by increased medication and follow up appointment compliance primarily. Another measure will be analyzing the frequency of rehospitalization of the patients enrolled in the program compared in the first twelve months. The need to present the program to leadership by the department of nursing of the psychiatric department will determine the project’s viability. Research studies mentioned will be included in the proposal to identify the benefits of the community’s seriously mentally ill population. Again, the mHealth smartphone FOCUS application results in patients’ reports of feeling supported at the moment, receiving medication reminders, coping strategies, and connection experienced as helpful to their recovery (Geneva et al., 2019). The cost of the program will be analyzed against the costs of frequent medication and follow up compliance by the seriously mentally ill population.
