Limitations

Although a capstone project may be associated with a number of strengths and benefits, unlocking this kind of a dissertation requires one to overcome a set of limitations. Some of the limitations that I identified when completing this capstone project included: smaller sample size in one agency, inability to apply to the current EMR system, rampant staffing challenges, and the lack of a better understanding of the importance of cancer treatment-related high-risk signs and symptoms management. The first limitation is the small sample size. Greeff et al. (2021) argued that a small sample size can undermine the reliability and credibility of a survey’s findings due to the occurrence of a higher variability that may contribute to bias. Moreover, the difficult associated with extrapolating the findings that involved a study with too small a sample may affect the quality of claims made in the realization of valid conclusions. In this capstone project, the sole focus on the Emergency Department at Mount Sinai West Hospital may have undermined generalization of the results, and utility of findings in other different healthcare settings.

The second limitation is the inability to apply the current EMR system. Blobel (2018) explained that although the electronic medical record is steadily emerging as an enabling technology capable of enabling quality improvement, improving care coordination, and facilitating data-sharing, there are many barriers to the adoption EMRs by healthcare practitioners. At the facility, some of the barriers to the adoption of EMR system at the identified healthcare facility included: high capital cost, lack of skilled resources for adoption and support, inability to redesign workflow and clinical processes in line with the current technology systems, concerns over the detrimental unplanned effects of EMRs, lack of incentives and inadequate data exchange. Others included concerns about patient acceptance, general reliability of the technology, concerns about privacy and security, and absence of adequately skilled personnel. Another key limitation was the staffing crisis. At the facility, a number of issues related to staffing included the lack of well-trained nursing staff and inadequate well-trained supportive oncology staff. Ding (2020) argued that lack of proper staffing can be a major barrier to implementing an EMR system, and hence limited staff resources could be a key factor behind the resistance to change from the staff personnel, leaders and managers of healthcare facilities. As such, strong emphasis must be placed on mobilizing the relevant staff resources, scheduling the EMR implementation over an extended period of time to avoid burnout, and selecting a more reliable vendor (Janssen et al., 2021).

Another key limitation of the capstone project is the complexity associated with management of cancer-related distressed in the sampled patients and survivors. Giannoulopoulos (2020) pointed out that disease and treatment-related complications in cancer patients should be considered a fundamental element of survivorship care. Despite the fact that many high-risk patients are exposed to a myriad of continuous therapy along the disease trajectory, a majority of healthcare providers have not been able to offer sufficient in-house training resources aimed at enabling their staff to understand the critical importance of cancer treatment-related high-risks signs and symptoms management. Notwithstanding the short period of the capstone project, the management of disease- and treatment related complications in patients with cancer emerged as one of the key steps to enhancing healthcare delivery in line with the established clinical guidelines and best practices.
