Social Determinants of Health Essay Discussion Paper

Introduction

According to Hill-Briggs et al. (2021), social determinants of health refer to the condition in which people are born, grow, live, work, and finally die. The distribution of money, resources, and power at the local, national, and global levels shapes such circumstances. In this case study, we explore the social determinants of health affecting an 85-year-old window facing several health issues. As her Nurse Practitioner (NP), understanding the social determinants is significant for fitting a far-reaching medical services plan that records her remarkable day-to-day environment, restricted mobility, and social support network. Social Determinants of Health

  1. Transportation- Depending on public transportation and encountering challenges while attending the church because of temperamental transport plans during unfavorable weather conditions influences her social commitment and admittance to fundamental administrations. Developing strategies to help her transportation needs, such as investigating nearby senior transportation programs, might be helpful (Alderwick & Gottlieb, 2019).
  2. Living Situation- The window’s choice to move into a first-floor “cave” because of trouble with stairs features the meaning of her everyday environment. As her nurse practitioner, ensuring her well-being necessitates addressing accessibility, fall risks, and home safety (Palmer et al., 2019).
  • Social Support- With no living child except a niece living around, her social support network is vital to her consideration. As her NP, cultivating correspondence with her niece, teaching her about the prescription routine, and including her in medical care choices will add to improved health outcomes (Alderwick & Gottlieb, 2019). Planning and Coordination as a Nurse Practitioner

  1. Mobility and Safety- the main priority here would be addressing this woman’s limited mobility and the fall risk associated with her condition. This can be facilitated by recommending assistive mobility devices, and encouraging exercise will form part of the care (Nutbeam & Lloyd, 2021).
  2. Medication management-it is important to ensure she receives the right medications considering her medical conditions, age, and gender. As her NP coordinating with her healthcare team to help prove pain management, suggesting home modifications for general Safety and utilizing analgesics will be the priority.
  • Nutrition and Dietary Support- Given her limited shopping for food, I will survey her nutritional status and, if vital, team up with a dietitian to give fitted dietary recommendations or consider wholesome enhancements to help her health(Nutbeam & Lloyd, 2021). 1. Accessibility of Healthcare- I will try giving available medical care options to her, like home visits or telehealth conferences, to oblige her transportation challenges and guarantee predictable clinical subsequent follow-ups.

Conclusion

As a Nurse Practitioner, understanding and tending to the social determinants of health for this situation study is fundamental for giving comprehensive and patient-focused care. I can create a comprehensive healthcare plan that meets her nutritional needs, ensures her access to healthcare services, promotes social engagement, and optimizes medication management by considering her living situation, transportation issues, and social support. This elderly woman’s improved health and quality of life will further benefit from her collaborating with her niece and including her in decision-making. Lesson 3: Discussion

Case Study

In general, women live longer than men. Consider the following factors and identify the potential impact on the pharmacological regimen for an 85-year-old, widowed, Social Security-dependent female with arthritis in her hands and impaired eyesight due to cataracts. She needs to walk with cane due to chronic bursitis in her left hip. She lives in a two story home, but has moved into a first floor “den” as her sleeping room to avoid climbing the stairs. She no longer drives but is adept at catching the bus, which stops a half block from her house. She has been active in her church, but only attends about two Sundays a month, avoiding the coldest and rainiest weather because the bus schedule is not always reliable on the weekends. She takes the bus to the grocery store, buying only the essentials she absolutely needs due to the bus ride. She has no living children, but has a niece who lives in town and calls her from time to time. The niece has three active children who come with her when she visits once a month.
