Further studies show that over 60% of community-dwelling patients discharged from healthcare facilities experience medication-related problems. The inappropriate use of medications results in adverse outcomes, leading to about 10% of hospital admissions relating to medication challenges (Kefale, Degu & Tegegne, 2020). Research on the US population shows that about 50% of admissions are preventable. The increased healthcare conditions requiring medications and multiple comorbidities place people at risk of MRPs. The database aims to achieve the following goals:

A number of objectives will guide the system developer. The developer is supposed to have defined all the database components that will make up the final system by the end of the second week after its initiation. By the end of the month, the developer should have developed a database design. The database should be completed within two months.

I will follow the steps of system development to achieve the goals and objectives. I will perform research to identify the data types that will be included in the database. I will also identify all the reports generated from the database. Second, I will define all the users of the database. I will then develop a design proposal to guide the database development process. I will then input existing data and test the system. Potential barriers to achieving the goals and objectives include insufficient resources and inaccuracies in the collected data. Data collected through surveys may be inaccurate due to the inability of individuals to recall their past drug utilization details. This interferes with the reliability of the database in generating reports that reflect the actual scenario among populations.

Communication Protocols

The communication protocols used in the database include interplanetary file systems, blockchain-based framework, and Fast Healthcare Interoperability Resource (Fang et al., 2021). The interplanetary file system allows for storing and sharing patient records in a distributed file system. Fast Healthcare Interoperability Resource also allows for the exchange of electronic health records. The blockchain communication protocol is an interoperability protocol that allows for reliable data transportation across different blockchains (Bayılmış et al., 2022). Addressing the challenge of medication-related problems in populations requires collaboration among healthcare practitioners, healthcare facilities, health agencies, legal bodies, and the public. Communication among the various parties is key to attaining the desired goals. Therefore, the development of a database will allow for the effective transfer of information among stakeholders to facilitate decision-making processes. The economic and technological requirements of the database make it feasible.
