Scenario: A friend calls and asks you to prescribe a medication for her. You have this autonomy, but you don’t have your friend’s medical history. You write the prescription anyway.Advanced Pharmacology Example Paper

Scenario used:
Pharmacokinetic and pharmacodynamic factors altered his or her anticipated response to a drug:
My experiences with hydrochlorothiazide over the last 5 years of my career has been a positive outcome for my patients within the emergency room setting. Anticipated responses to the treatment of hypertension and edema are decreased blood pressure from a hypertensive crisis diagnosis, increased urinary output to eliminate edema and blood pressure parameters are within a safe range. Patients showed improvement with blood pressures and in congestive heart failure patients lungs sounds by elimination of edema. Pharmcodynamics drug movement throughout the body (Rosenthal, L. D., & Burchum, J. R., 2021). It reduces blood volume by acting on the kidneys to reduce sodium (Na+) reabsorption in the distal convoluted tubule.Advanced Pharmacology Example Paper
Consider factors that might have influenced the patient’s pharmacokinetic and pharmacodynamic processes, such as pharmacogenetics: (gender, ethnicity, age, behavior)

Age: A greater blood pressure reduction and an increase in side effects may be observed in the elderly (i.e., > 65 years) with hydrochlorothiazide. Starting treatment with the lowest available dose of hydrochlorothiazide (12.5 mg) is therefore recommended (fda.gov, n.d.).
Ethnicity: Larger and longer prospective studies are still required to further clarify the long-term safety and metabolic impacts/effects of thiazide diuretics in Nigerians with essential hypertension (Olamoyegun, M., Kolawole, B., & Ajayi, A., 2017).`
Gender: The relationship between the ACE I/D polymorphism and antihypertensive response to a standard dose of HCTZ differs significantly between women and men. Because the D-allele was associated with significant, co-dominant increases in serum ACE activity in both genders, the gender-specific effects of the I/D polymorphism on BP response to HCTZ may be mediated subsequent to the enzymatic generation of angiotensin II (chwartz GL, & Turner ST, et. al., 2002).
Behavior: No information on available at this time.
Think about a personalized plan of care based on these influencing factors and patient history in your case study:Advanced Pharmacology Example Paper
Patient is prescribed Hydrochlorothiazide 12.5 mg p.o daily with a 3 month supply. She will monitor her daily urinary output, weights, pulse rate, blood pressures daily, evaluate edema and keep a record of it. Follow-up appointments every 3 months when refills and new prescription are due. Pt is to report all adverse reactions and side effects as warranted.
