Background
When the novel coronavirus pandemic came it affected all parts of the world. Healthcare settings are some of the worst affected because that is where sick persons congregate. King stone Healthcare in California is one such healthcare organization that has been hit particularly hard. Apart from the facility experiencing rising numbers of ED admissions due to Covid-19, its staff also started getting infected with the virus compounding the problem of staff shortage. The facility however took early steps to train the staff on the proper use of personal protective devices or PPEs. It also undertook to test the staff for Covid-19 at least every week. These measures proved successful in lowering the infection rate among the nursing staff. Clinical Problem Statement
Covid-19 infection rates have been rising all over the country with deaths increasing and new variants emerging despite preventive measures. Those who are more vulnerable have been seen to be those who are elderly and have pre-existing conditions such as heart disease, hypertension, diabetes mellitus, or chronic obstructive pulmonary disease (COPD) amongst others. At Kingston Healthcare, these represent the largest number of Covid-19 patients seen in increasing numbers. Unfortunately, deaths from the viral infection have been high despite giving the best supportive therapy and oxygen supplementation. There is need to stop and reduce the spread of Covid-19 within the community served by Kingston Healthcare. The number of patients seen at the facility with Covid-19 also needs to drop.

Purpose of the Change Proposal
In relation to the provision of quality patient care in a changing healthcare environment, the purpose of this change proposal was to use the available evidence to design interventions that would be cost-effective and practical in enabling an entire community around Kingston Healthcare to avoid getting infected with the Covid-19 disease.
PICOT Question
Among persons at risk of infection with the novel coronavirus (P), does the consistent use of face masks, social distancing, and handwashing together with vaccination (I) compared to doing nothing (C) result in a reduced rate of infections (O) for a period of one calendar year (T)?
Literature Search Strategy Employed
The literature search strategy employed comprised of searching reputable research databases for peer-reviewed evidence in support of the interventions stated in the PICOT statement. These included Embase, CINAHL, PubMed, and Cochrane. Key search words and phrases were used and connected with the Boolean operator ‘AND’. Then the year of publication filter was applied and this could only be within the last five years. Evaluation of Literature
A total of eight studies were selected for their evidentiary value in supporting the change proposal and interventions. They were Alimohamadi et al. (2020), Bundgaard et al. (2020), Chu et al. (2020), Harapan et al. (2020), Li et al. (2020), Najnin et al. (2019), Oladeji et al. (2020), and Yuan et al. (2020). A good number of the studies are systematic reviews and meta-analyses and hence provide a high degree of evidence.
Applicable Change or Nursing Theory Used
The change or nursing theory used to guide the process of change at Kingston Healthcare was Roger’s Diffusion of Innovations (DOI) model (Dearing & Cox, 2018).
Proposed Implementation Plan and Outcome Measures
The proposed implementation plan was to go through the change theory stages for Roger’s DOI model with the proposed interventions. These are (i) giving knowledge about the problem and interventions; (ii) persuading the intended audience to adopt them; (iii) allowing them to make a decision to adopt or not; (iv) implementation of the interventions after acceptance, and (v) confirmation of the change recommendations into practice. Outcome measures will be the post-intervention Covid-19 rate of infection and the number of persons vaccinated against Covid-19.
How EBP was Used to Create the Intervention Plan
The intervention plan was created out of the process normally followed to build EBP knowledge. That is the process of clinical inquiry using the PICOT model. It is a foolproof method that only ends up with solid change recommendations backed by incontrovertible scholarly evidence supporting change recommendations.
Evaluation Plan
Success or failure of the project will be evaluated by examining the medical data for Covid-19 patients treated at Kingston Healthcare in California. For instance, the total number of patients and others vaccinated will give an indication of the success of failure of the process.
Potential Barriers to Plan Implementation
Some of the potential barriers to the implementation of this change plan include scarcity of financial resources for logistics, a shortage of nurses to carry out interventions, and the need to store the current vaccines at deep sub-zero temperatures.
