In this project, the goal is to determine some of the factors leading to patient-safety risks, identify evidence-based solutions, expand on the role of nurses in coordinating care, and evaluate the involvement of health stakeholders in the implementation of a quality improvement initiative. Factors Contributing to Increased Risks in Patient Safety

One of the primary factors that increase and exacerbate patient-safety risks is the limited understanding among patients. Most patients do not comprehend their present conditions, treatment options, and available care plans. As a result, this could lead to misuse of medications, misinterpretation of symptoms, and not recognizing warning signs. More so, it could lead to development of preventable adverse situations and thus create a potentially life-threatening situation (Patient safety, 2023). Lack of adequate patient education compromises informed consent and poses certain ethical and legal ramifications for health personnel. There are other factors that further lead to increased risks in patient safety as highlighted by the World Health Organization (WHO) and include human factors and behavior, which are breakdown in communication among healthcare workers in a healthcare team and with patients and their families (Patient safety, 2023). Patient harm in a healthcare setting can occur due to safety breaks and is pervasive and problematic and could occur in all settings and at different levels of healthcare provision. Addressing the issues related to inadequate health-care education would require implementation of evidence-based solutions. As Degu et al. Nurses Coordination for Patient Safety and Cost Reduction

Nurses have a critical role to play in coordinating care in a way that increases patient safety and lowers costs. As Cullen et al. (2021) posits, nurses design, implement, and indulge in care coordination processes and practices aimed at improving patient outcomes and lower costs. As a result, they demonstrate effectiveness of nurse-led and patient-centered coordination of care. They evaluate the patient’s knowledge on their ailing status and identify if patients have any information needs. More so, they collaborate with interdisciplinary teams for educational support and evaluate their adherence to treatment plans. In their capacity as healthcare providers, they identify any potential complications and contribute to bettered patient outcomes and overall cost-effective healthcare provision. Care coordination entails the management of patient care between people. Errors could occur in the process during prescribing, transcribing, supply dispensing, administration, monitoring, and documentation (Russ-Jara et al., 2021). When patients are misinformed or fail to comprehend the message provided effectively, they are likely to have medication hazards such as consumption of duplicates, confusion on dosages, and drug-drug interactions. As a result, nurses are needed to coordinate care with other health professionals and ensure that these harms and mishaps are avoided for the betterment of patients. Involvement of Healthcare Stakeholders

The success of quality improvement initiative centered on ensuring that patients have adequate education is centered on effective collaboration with different stakeholders. Mainly, these stakeholders include physicians, pharmacists, and allied health personnel.  Communicating with advisors is needed to ensure that the informed consent processes are aligned with the legal recommendations.  According to Cullen et al. (2021), there is a need for strategic and formal involvement of patients and the public to create a culture of best practice in patient care and this requires a vision and investment in patient education. A strategic framework is needed to ensure that patient education is embedded throughout all healthcare departments. Patients should have the appropriate guidance and support for sustainable healthcare practices.

Monitoring and evaluation

There are certain metrics needed to evaluate the effectiveness of patient education in a healthcare setting. Some recommended monitoring and evaluation practices involve conducting regular audits, implementation of feedback mechanisms, and having a quality improvement initiative. Continuous improvement techniques are needed to determine areas in need of improvement and ensure there are sustained enhancements in patient education. Some of the key principles of monitoring and evaluation is participation of stakeholders, relevance, efficiency, effectiveness, sustainability, ethical considerations, and continued improvement. Patient education in healthcare is important for enhancing patient safety, cost reduction, and fostering positive patient outcomes. The proposed quality improvement initiative is improving patient education and is based on evidence-based solutions, coordination of nursing care, and involvement of different stakeholders for a collaborative approach of enhancing bettered patient outcomes. Implementation of this plan requires health organizations to enhance patient educations, empower others, and ultimately attain the goal of quality healthcare.
