The primary diagnosis for the patient is Cholelithiasis. Cholelithiasis, also known as gallstone ileus, is an intestinal obstruction caused by one or more gallstones becoming impacted within the bowel’s lumen (Ball et al., 2018). This condition is more common in the elderly population. Abdominal pain, especially RUQ pain, is a prominent symptom of cholelithiasis and it is associated with illnesses such as diabetes and cardiovascular disease. The patient has a history of hypertension and Type II Diabetes Mellitus. The impacts and dis-impacts caused by the tumbling of gallstones in the gastrointestinal tract cause obstruction which consequently causes frequent diarrhea when there is intermittent abdominal distention. The patient reported feeling nausea and having vomited. Nausea, vomiting, and right upper quadrant pain are major symptoms strongly associated with Cholelithiasis (Woo et al., 2015). Treatment with surgery and medications such as ursodeoxycholic acid (UDCA) and chenodeoxycholic acid (CDCA) that dissolve gallstones. These drugs alter the composition of bile, improve bile flow, decrease secretion of cholesterol by the liver into bile, decrease intestinal absorption of cholesterol, and dissolve gallstones. Although a combination of Chenodeoxycholic Acid and Ursodeoxycholic Acid is effective in dissolving radiolucent gallstones, laparoscopic cholecystectomy remains the standard treatment for symptomatic gallstones (Hassler et al., 2022). Treatment with bile dissolving agents also takes too long to effectively clear the gallstones. Contrary, laparoscopic cholecystectomy is a safe, feasible, and successful procedure with a low readmission rate and associated minimal complications.
