Thank you for your submission of Assessment 1. You are off to a good start. I like that your focus of change is about medical errors and inadequate blood sugar control in diabetic patients. However, this assessment is asking you to address a local or regional setting such as a county or state health care system or program that should be the focus of change. You provide relevant sources but there are many facts and claims that are missing supporting evidence and sources.

See my comments. Please highlight your changes to capture your revised work.

I am confident that you can achieve Proficient or Distinguished if you incorporate my feedback.

COMMENTS:

Sadeena, you provide a good discussion about medical errors and inadequate blood sugar control in diabetic patients. However, this assessment is asking you to address a local or regional setting such as a county or state health care system or program that should be the focus of change. Be specific with the organization you are speaking to within the paper to identify the local or regional area.

How can you address medical errors and inadequate blood glucose control in diabetic patients as a focus of change in your local or state health care system or program???

I would like you to provide clear expectations for improvements substantiated by credible evidence. What improvements do you expect as a result of the change? What evidence supports your position?

How do you propose to reduce burden and improve the quality of life of diabetic patients??

CRITERIA 2

Define desirable outcomes, including who will pay for care and factors limiting achievement of those outcomes.COMPETENCY

Compare the effects of different health care finance models and policy frameworks on resources and patient outcomes.

NON_PERFORMANCEDoes not define desirable outcomes.BASIC Defines desirable outcomes, without including responsible payer or limiting factors.PROFICIENTDefines desirable outcomes, including who will pay for care and factors limiting achievement of those outcomes.DISTINGUISHEDDefines desirable outcomes, including who will pay for care and factors limiting achievement of those outcomes. COMMENTS:

You state that the desired outcome of the proposed change is reducing the burden and improving the quality of life for patients with T2DM by increasing the accuracy of medical prescriptions and caring for the patients.

However, how will reducing the number of healthcare visits, medications, and complications associated with T2DM improve the overall health of patients???

Who will pay for care? What are the factors limiting in achieving those outcomes? Outcomes should be measurable and be the same outcomes that are addressed in the Healthcare System Comparative Analysis table which needs to be included. Please use the template for this assessment which includes the table.

What outcomes do you want to achieve with your proposed changes that you need to describe?? How would you measure quality of care???? suggestions would include: decreased ED visits, decreased readmissions, decreased LOS, decreased mortality and morbidity rates, patient satisfaction???

CRITERIA 3

Explain why specific changes will lead to improved outcomes.COMPETENCY

Develop proactive strategies to change the culture of the organization by incorporating evidence-based practices.

NON_PERFORMANCEDoes not describe expected improvements in outcomes.BASIC Describes expected improvements in outcomes, or provides no rationale for expectations of improved outcomes.PROFICIENTExplains why specific changes will lead to improved outcomes.DISTINGUISHEDExplains why specific changes will lead to improved outcomes. Draws well-substantiated conclusions that are in line with reasonable expectations.

COMMENTS:

You discuss the complications of medical errors. However, this criteria is asking you to explain how specific changes will lead to improved outcomes??? What are the expected improvements in outcomes? What evidence can you use to justify your conclusions? Are your expectations reasonable within the existing health system?  Challenges And Opportunities Facing Health Care Essay Paper

Articulates insightful lessons learned from the analysis that have clear implications for U.S. health care.

COMMENTS:

You state that in the last two decades, the prevalence of T2DM in Canada and United Kingdom have significantly increased. You only discuss that In UK, medical errors that T2DM patients face are addressed through the National Diabetes Audit. The audit collects data on diabetic patients’ experience with healthcare providers in order to improve quality of care (Elliott et al., 2018). In Canada, the government has introduced the Action Plan for Diabetes, which calls for a coordinated national approach to improve the care of diabetic patients. One of the key elements of this plan is improving communication between healthcare providers and patients. Proper communication and coordination have reduced many medical errors that are committed when caring for diabetic patients (Shojania & Dixon-Woods, 2017).  Challenges And Opportunities Facing Health Care Essay Paper

However, the Healthcare System Comparative Analysis table needs to be included. Please use the summary report template provided for this assessment. I would like to see the Healthcare System Comparative Analysis table analyze what these countries implemented to produce positive measurable outcomes that should be identified in Criteria 2.

What lessons are you able to draw from each example that you could apply in your system?

You determine the financial and health implications of medical errors. associated with the proposed changes to a health care system or program.  As of 2020, statistics on financial cost of managing diabetes showed that the total annual cost of managing diabetes due to medical errors is $245 billion, with a total economic impact of $27.4 trillion (Alqenae et al., 2020).

However, this criteria is asking you to determine the financial and health implications associated with a specific proposed change to a health care system or program. Please address the implications of making the changes and not making the changes. What are the likely costs and benefits for individuals and the community at large? What are the potential short and long term effects?
