Benchmark-Risk Management Program Analysis-Part One

A Summary of and the Rationale of the Selected Program

I am a nurse working in a primary healthcare facility part-time. As a nurse working in a primary care facility, the risk management program that I have selected is specifically designed for primary care settings improvement or management. The specific one selected, therefore, is the Primary Care Risk Management Program (PCRMP). In the United States, this program is offered by The Doctors Company (Ericksen, 2004). Dragic et al., (2018) explain that this particular program is designed to help primary care providers identify and manage risks associated with their practice making the specific audience for the program clinical nurses. Included in the program are a variety of resources such as online courses, webinars, risk assessments, and practice guidelines. Additionally, the PCRMP provides access to risk management consultants who assist with specific risk management issues. Some of the risk management topics addressed by the program include medical errors, communication breakdowns, documentation, informed consent, and patient safety. This is important for a healthcare organization and more for clinical nurses as the program emphasizes the importance of proactive risk management, and provides tools and resources to help providers prevent and mitigate risks before they become serious problems.

My choice of selecting PCRMP was driven by several motivations. One, the program is specifically designed for primary care settings, which is the anticipated professional arena in this scenario. Two, The Doctors Company is a well-established and respected medical malpractice insurance provider, and its risk management program is built upon its extensive experience in the field. Last, the PCRMP is comprehensive and practical, providing primary care providers with a range of tools and resources to help them identify and manage risks in their practice. Organization to Which It Applies

The healthcare organization to which this plan applies is a primary care facility like the one I currently work on a part-time basis. In this setting, risk management plays a critical role in ensuring high-quality patient care as primary care providers are responsible for managing a wide range of health issues, and their patients often have complex medical histories and multiple chronic conditions (Berkman et al., 2015). Given this complexity, clinical nurses working in primary care organizations are at an increased risk of making medical errors, triggering communication breakdowns, and other issues that can negatively impact patient outcomes. In their article on the application and importance of the PCRMP, Dragic et al., (2018) note that the role of risk management is to identify and manage these risks proactively before they cause harm to patients. More importantly, risk management involves a range of activities such as risk assessments, patient safety protocols, documentation and communication standards, and quality improvement initiatives. With the implementation of a comprehensive risk management plan, primary care providers reduce the likelihood of adverse events and improve patient safety and outcomes. Furthermore, risk management helps primary care providers identify areas for improvement and implement changes that enhance the quality of care they provide to their patients. Standard Administrative Steps and Processes versus the Doctors Company’s PCRMP

In a typical healthcare organization, the standard administrative steps and processes in a risk management program typically involve the five key steps. Risk Identification is the first step and involves identifying risks in any risk management program. This involves reviewing medical records, incident reports, and other data to identify potential risks and hazards that could lead to patient harm. The second step is risk assessment. Once risks have been identified, the next step is to assess their likelihood and severity which involves determining the probability of an adverse event occurring and the potential impact it could have on patients and the organization. The third step is risk control which refers to the process of implementing measures to mitigate identified risks. Risk control involves implementing new policies and procedures, providing staff training, improving communication, and using technology to reduce the likelihood of adverse events. Risk financing, the fourth step, involves determining how to finance the costs associated with managing risks and includes insurance, self-insurance, and other financial arrangements. This is then followed by step number five which is Claims Management. In the event of an adverse event, the claims management process involves investigating the event, determining liability, and managing any resulting claims or lawsuits.

In comparison, The Doctors Company’s Primary Care Risk Management Program also follows a similar set of administrative steps and processes, but with some additional features that are tailored specifically to primary care settings. The first step in the PCRMP is online education and training. The program provides online education and training resources to help primary care providers understand and manage the unique risks associated with primary care practice. The second step involves the provision of practice guidelines. The PCRMP provides practice guidelines that are specifically designed for primary care settings, covering topics such as chronic disease management, screening and prevention, and care coordination. This is then followed by the third step which is risk management consultation. The program provides access to risk management consultants who help primary care providers with specific risk management issues. The last and final step in the PCRMP is root cause analysis. PCRMP emphasizes the importance of conducting root cause analyses following adverse events, to identify the underlying causes of the event and implement changes that reduce the likelihood of similar events in the future. The Key Agencies and Organizations

In the United States, several key agencies and organizations regulate the administration of safe health care, each playing a different role in the risk management oversight process. The first is the Centers for Medicare and Medicaid Services (CMS). CMS is responsible for overseeing the administration of federal healthcare programs, including Medicare and Medicaid (Wadhera et al., 2020). CMS plays a role in risk management oversight by setting standards and regulations for health care providers that participate in these programs. The second organization is the Joint Commission (TJC). According to Arnetz (2022), TJC is a nonprofit organization that accredits and certifies healthcare organizations and programs in the United States. Additionally, TJC plays a role in risk management oversight by setting standards for patient safety and quality of care, and by conducting on-site surveys to evaluate compliance with these standards. The third is the Agency for Healthcare Research and Quality (AHRQ) which is a federal agency that supports research and initiatives to improve the quality and safety of healthcare in the United States (Berkman et al., 2015). AHRQ plays a role in risk management oversight by funding research on patient safety and quality improvement, and by providing resources and tools to help healthcare providers identify and manage risks. Compliance with Corresponding MIPPA-Approved Accrediting Body

PCRMP has been explained as a comprehensive program designed to improve patient safety and mitigate risks in primary care settings and assists healthcare organizations ensure compliance with various accrediting bodies, including those approved by MIPPA, by addressing privacy, healthcare worker safety, and patient safety concerns in a handful of ways. Firstly, PCRMP ensures compliance with HIPAA regulations and other privacy laws by providing training and guidance to healthcare workers on handling and protecting patient health information. This includes best practices for maintaining confidentiality, securely storing and sharing patient information, and ensuring that patient consent is obtained for any disclosures or uses of their information. Secondly, it helps improve healthcare worker safety by addressing potential hazards in the workplace, providing training on safe practices and procedures, and promoting a culture of safety. Thirdly, it leads to an improvement in patient safety by identifying and mitigating risks in primary care settings such as medication errors, diagnostic errors, and communication breakdowns.

Proposed Recommendations

There are three important recommendations I would make. One, there should be regular training and education for healthcare workers on privacy and security policies and protocols. Two, the organization should focus on implementing a system for reporting and tracking workplace injuries and near-misses to identify and address potential health and safety risks. Last, it is important to the establishment of a process for conducting regular risk assessments and audits to identify areas for improvement and implement necessary changes.
