Client Benefiting from Hospice or Palliative Care Referral
I encountered a patient with advanced heart failure and multiple comorbidities. Recurrent hospitalizations, frequent pain, and significant emotional distress accompanied her declining health. A recommendation for palliative care or hospice could have been highly beneficial. She may have benefited from both services by receiving complete pain management, emotional support, and help transferring to end-of-life care.
Approaching the Conversation with the Client
I would express my concern for the patient’s well-being. I would introduce the concept of hospice and palliative care. This would involve emphasizing their focus on enhancing her quality of life and minimizing her suffering. I would listen attentively to her fears and preferences, ensuring she understands that her choices are valued and respected. By addressing her physical and emotional needs, I aim to provide the patient with the utmost care and dignity during this challenging phase of her life.
