Differences in cultural beliefs, subcultures, religion, ethnic customs, dietary customs, language, and a host of other factors contribute to the complex environment that surrounds global healthcare issues. Failure to understand and account for these differences can create a gulf between practitioners and the public they serve.

In this Assignment, you will examine a global health issue and consider the approach to this issue by the United States and by one other country.

To Prepare:

Review the World Health Organization’s (WHO) global health agenda and select one global health issue to focus on for this Assignment.
Select at least one additional country to compare to the U.S. for this Assignment.
Reflect on how the global health issue you selected is approached in the U.S. and in the additional country you selected.

Part 1: Global Health Comparison Matrix

Focusing on the country you selected and the U.S., complete the Global Health Comparison Matrix. Be sure to address the following:

Consider the U.S. national/federal health policies that have been adapted for the global health issue you selected from the WHO global health agenda. Compare these policies to the additional country you selected for study.
Explain the strengths and weaknesses of each policy.
Explain how the social determinants of health may impact the global health issue you selected. Be specific and provide examples.
Using the WHO’s Organization’s global health agenda as well as the results of your own research, analyze how each country’s government addresses cost, quality, and access to the global health issue selected.

Global health issue: Maternal mortality The United States has among the highest maternal mortality rates, for a developed country. The maternal mortality rate since 1987 has steadily increased standing at 16.7 deaths in every 100,000 births in 2016 (Hayes & McNeil, 2019). In Canada, the maternal mortality rate currently stands at 8.3 death for every 100,000 live births, as at 2018 (Statistics Canada, 2018). This figure is lower than the U.S’

In 2015, the Canadian government committed $3.5 billion to enhance the health and rights of women and children around the world, including in Canada. This fund is aimed at enhancing maternal health and child health by providing immunizations and improving their nutrition, especially those in the rural areas.
Strengths and Weaknesses A major strength with this policy is that it commits about $9 million annually to the HHS to fund its programs in enhancing rural maternal health especially for the mothers with inadequate knowledge on maternal care.

A major weakness is that it only focuses on rural mothers, considering that mothers in urban areas are also susceptible to maternal mortality.

A major strength is that the funds provided through the policy are instrumental in reducing maternal mortality because they fund programs that cater to needs such as proper nutrition and obstetrics care for the mothers.

A major weakness with this policy is that it does no focus solely on Canada but also includes other countries; hence, may not be as effective in reducing maternal mortality especially among the indigenous Canadians in the rural areas

Social determinants of health A major social determinant that affects maternal mortality is the availability of the resources to meet daily needs. Some of these needs include proper nutrition, safe housing, and medication. For instance, if a mother lives in a poor neighborhood that is unsafe, they may have difficulties accessing healthcare facilities in the middle of the night because of the insecurity, hence, leading to maternal mortality. Another social determinant that could contribute to maternal mortality is access to healthcare services. The United States and Canada have higher mortality rates among the rural mothers with the main reason being access to health services.

Addressing costs, quality, and access to maternal health Under the ACA, the U.S provides maternal services for mothers as essential services. The government has lowered maternal health costs for mothers so as to reduce morbidity and mortality. Additionally, through the Triple Aims that cover all healthcare services, maternal health quality is improved. The Canadian government provides maternal health services for its patients through public health units funded by the government. The Canadian government also provides its citizens with access to the Prenatal Nutrition Program, which is a community based program that helps pregnant women with information on nutrition to sustain their pregnancies.
Maternal mortality and global population A high maternal mortality rate would reduce the global population because most times, the mother and child are often at risk and succumb to the complications during or after child birth.
Maternal mortality and role of the nurse Maternal mortality in both countries would enhance the nurse’s role, especially those who specialize in midwifery to increase their professional abilities so that they can guide mothers on precautions to take before and after birth. It also reinforces the role of the nurse as a caregiver since some mothers die from complications after child birth due to negligence from the clinicians.

 

Part 2: Plan for Social Change

To advocate for the incorporation of a global perspective into local practice as a nurse leader, I would seek to hold meetings with the organization’s shareholders and do presentations on the various factors that lead to maternal mortality in the country. I would then use these perspectives and apply them with regards to the healthcare facility in which I work, to conceptualize the information.
