The outcome measures in addressing the health disparities among homeless African Americans would include the following:
Finances to Implement Plan
One of the most reliable sources of finance for this plan would be the state and county level governments. This plan needs to be localized in specific areas, since it is easier to deal with the homeless per locality, as opposed to federal level. Therefore, the first source of finance should be the state and county governments. The second source should be a call for well-wishers to offer their donations to the cause. Another option is to approach the corporate entities within the locality to donate to the program, as part of their social responsibility.
