Personal Goals for this Course 

My nursing specialty is Adult-Gerontology Primary care nurse practitioner. Upon completing this course, I will integrate theories, experiential knowledge, and research-based evidence into my advanced adult gerontology nurse practice. I also hope to acquire advanced practice nursing skills in providing adults and older adults with high-quality nursing care. Furthermore, I hope to acquire the knowledge and skills to manage the adult gerontology population seeking medical care in a complex healthcare system. I also plan to use evidence-based practice, policy development, leadership, and education acquired during the program in developing adult-gerontology advanced practice nursing practice. Integrate relationships between social, cultural, political, and economic issues and health care delivery for adults throughout their lifespan and their families. Population to work with during Clinical Practicum, Experience Working with them, and the Comfort Level in Working with this population

I will work with the adult population across the lifespan during clinical practice. I will attend to adults presented to the clinic with cardiovascular diseases, including coronary heart disease, cerebrovascular disease, hypertension, and rheumatic heart disease. According to Aguayo et al. (2018), cardiovascular disease prevalence is higher in geriatrics than in general. I have experience working with adult patients in long-term care and hospital inpatient in the cardiac care unit. Currently, I work at the Hematology/oncology infusion center, where I attend to adults across the lifespan. It has been great working with adults in different healthcare settings. Due to their cooperation, I am comfortable working with these patients in various stages of treatment, including assessment, diagnosis, and management of diagnosed health conditions. I focus on providing them with high-quality nursing care to achieve treatment goals.

Skills that you think you have adequately developed up to this point

Up to this point, I have enhanced my interactive and collaborative skills. I can collaborate with interdisciplinary team members during care delivery. Collaboration among team members during care delivery contributes to high-quality patient care (Rosen et al., 2018). During care delivery, interdisciplinary team members, including primary care providers, nutritionists, and physiotherapists, use their knowledge, skills, experience, and competence to make decisions. Consequently, the adult gerontology population receives high-quality, safe, and effective health care, resulting in optimal health outcomes. Skills to be enhanced during this Clinical Experience

During this clinical experience, I intend to improve my cultural competence skills. According to Lee et al. (2020), cultural competence skills enable nurses to provide high-quality care to individual patients with diverse cultural and ethnic backgrounds. Therefore, cultural competence skills acquired during the course will enhance my understanding of the beliefs, values, and traditions of individuals from other cultural and ethnic groups. As a result, I will provide high-quality care to patients with different cultural perspectives. Additionally, patients will cooperate if their culture is acknowledged and respected during care delivery, resulting in positive health outcomes.

Any other thoughts or concerns relevant to this clinical experience

I am concerned with the quality of care for elderly adults with cardiovascular diseases. Studies indicate a high rate of hospital readmission and mortality among elderly adults with cardiovascular diseases (Noale et al., 2020). These adverse health outcomes indicate that elderly adults with cardiovascular diseases receive low-quality health care. Thus, cardiac units should focus on improving the quality and efficacy of healthcare received by these patients to optimize their health outcomes. Week 14: Self-Reflection

Were initial goals achieved during the semester?

This semester was highly educative. I met personal goals stated in the initial self-reflection during the semester. First, I improved my nursing skills, knowledge, and experience by completing weekly discussions and assignments. Additionally, I achieve all nine NONFP core competencies by working with my preceptors and other healthcare providers during the practicum practice. Furthermore, I improved my clinical guidelines knowledge by using established guidelines while dealing with different parents during the practicum.

Concepts about working learned during this semester

I learned various concepts about working throughout the semester. First, I learned the significance of establishing a good therapeutic relationship with patients at the beginning of treatment. Kwame and Petrucka (2021) reported the importance of a good-nurse relationship in achieving positive health outcomes. Therefore, I will establish a good relationship with adults diagnosed with cardiac and endocrine disorders in clinical practice. Additionally, I learned that collaborating with other healthcare providers results in optimal patient care.

Working with speciality populations in the future

I worked at the walk-in clinic/urgent care and primary care clinic during the practicum experience. I was attending to adult patients with cardiac and endocrine system disorders. As an adult gerontology primary care nurse practitioner, I intend to deal with adults diagnosed with cardiac and endocrine system disorders in the future. I will use the knowledge, competence, skills, and experience acquired during the clinical rotation to provide effective, safe, efficient, and high-quality care to adults with cardiovascular diseases and endocrine system disorders in urgent care and primary care clinics, resulting in positive health outcomes. Changes in professional goals during the semester

Clinical experience has changed my professional. At the beginning of the semester, I aimed to enhance my healthcare delivery skills. However, my encounters with my preceptors, physicians, other nurse practitioners, and patients changed my perception of nursing practice for the better. I learned that nursing is a unique and complex profession prioritising holistic patient care. Therefore, I focus on equipping myself with diverse skills essential for providing holistic patient care. As a nurse practitioner, I will show empathy and care to all my clients to achieve optimal patient care.

Clinical skills that improved during the semester

I improved my clinical skills throughout the semester. First, I acquired effective communication skills. As a nurse practitioner, I will use these skills to guide and instruct other healthcare providers in the clinical practice, achieving personal and organizational goals. Secondly, I acquired time management skills. As a nurse practitioner, this skill will result in the provision of timely and efficient patient care. Lastly, I improved my comprehensive physical examination skills. Incorporating this skill in my clinical practice as a nurse practitioner will result in the most appropriate diagnosis and treatment. Clinical skills requiring improvement

However, I need to improve some clinical skills. First, I should improve my X-ray interpretation skills. This skill is essential in my practice as a nurse practitioner since it will enhance the diagnosis process. Secondly, I should improve my conflict resolution skill. According to Alshehry (2022), conflict and disagreements in healthcare organizations result in adverse health outcomes. As a nurse practitioner, I will apply this skill in uniting other subordinates and maintaining harmony in the workplace to create a conducive working environment, resulting in positive health outcomes.

Concerns regarding the semester

My concerns involve the time allocated for the concepts covered during the semester. Most topics were broad and complex. Therefore, the course should be assigned more time to enhance learners’ understanding of various topics and concepts covered during the course.
