Released in 2010, the Institute of Medicine’s (IOM) report thoroughly evaluated the nursing workforce and provided recommendations on leading and advancing healthcare (Altman et al., 2016). The IOM report had and continues to directly impact nurses at the individual, organizational, and national levels. Primarily, the report’s goal is to enhance patient outcomes for diverse populations across lifespans, with nursing as the vital factor required to achieve the goal. Inherently, the IOM report details how changes in healthcare resulting from technology, patient demographics, and other factors impact nursing expectations and demands. Its recommendations reflect collaborative actions and policies that the profession should adapt to fit in the shifting practice environment.Key Messages that Structure IOM Recommendations.

Work of Robert Wood Johnson Foundation Committee Initiative that Contributed to the IOM report

In its work, the RWJF committee developed a vision that would seek to transform the healthcare system while considering the demands and limitations of the current care system. The committee imagined a future healthcare system that provided access to quality care to diverse populations in the US. Such a system would deliberately promote wellness, prevent disease, enhance health outcomes, and deliver compassionate care across the continuum. In the envisioned future, primary care and disease prevention were major drivers of the healthcare system. In 2008, the foundation launched a two-year initiative to assess and provide recommendations for achieving the envisioned healthcare system.Key Messages that Structure IOM Recommendations.

Key Messages that Structure IOM Recommendations

The IOM report provided four crucial recommendations. The first message was that nurses should be allowed to practice to their full degree of education and training. The second recommendation is that all nurses should accomplish higher educational and training levels through an enhanced education system that facilitates seamless academic progression. The third recommendation centered on nurses partnering with clinicians and other health workers in reshaping healthcare in the United States. The last suggestion was that efficient workforce planning and policymaking need improved data collection and enhanced information structure. The above four recommendations have significantly influenced nursing practice, education, leadership, and workforce development.Key Messages that Structure IOM Recommendations.

Nursing Practice

Operating effectively for nurses requires them to have an array of skills ranging from health promotion, disease prevention, palliative care, and care coordination within practice settings (Hassmiller, 2018). Such skills are crucial in meeting the dynamic health needs of the diverse American population. The IOM report warned that as a profession, nursing must change to meet the increasing demands resulting from the surge of an aging population, advancement in medical technology, and more health reforms.Key Messages that Structure IOM Recommendations.

Nursing Education

The IOM recommendations have also influenced nursing education by laying certain requirements that nursing students must achieve to be competent. Specifically, the recommendations highlighted that nurses should progress to higher degrees, and baccalaureate programs should be increased in nursing programs. Education systems must, therefore, offer tools required to enhance standards of quality care. Nurses must adopt continuous life-long learning that incorporates basic education, residency programs, and life-long competence.Key Messages that Structure IOM Recommendations.

Nursing Leadership

To be effective leaders, nurses must work in tandem with other healthcare professionals and be accountable for their contribution to patient care (DeWitty, 2019). To accomplish this goal, the IOM report proposed that leadership skills should be acknowledged as a core competence in all levels and be incorporated in the nursing curricula. Further, the report encouraged nurses to take their leadership skills on the policy level through patient advocacy.Key Messages that Structure IOM Recommendations.

Workforce Development

Nurses constitute the largest segment in the US workforce. Therefore, establishing an organized framework for obtaining workforce data is incredibly crucial. Such an approach will identify gaps in meeting patient needs while identifying inherent overlaps. The information will then allow future workforce planning to improve healthcare information infrastructure.

Role of State-Based Coalitions

State-based coalitions are organizations led by nurses and business partners that intend to drive change locally by transforming healthcare by building healthier communities. They are composed of representatives from nursing, businesses, health organizations, and education to implement IOM recommendations in their respective states. State-based coalitions advance the IOM messages by actively campaigning for actions to transform the profession.Key Messages that Structure IOM Recommendations.

New Mexico State Action-Based Coalition

The coalition in my state is a nurse-led group of community leaders who offerdeliberate guidance through teamwork and grass-root efforts with crucial partners to modify the way in which care is delivered and facilitate wellness to the population of New Mexico. The organization’s first initiative is working with different colleagues in the state to institute the IOM recommendations and cultivate a culture of health. New Mexico action state-based coalition addresses issues such as cultivating healthier communities, facilitating interprofessional collaboration, increasing nursing diversity, obtaining workforce data and fostering effective nursing leadership. The second coalition’s initiative is working collaboratively with public health nurse leaders to increase the number of nurses working with community partners at the community level. The above initiatives have significantly advanced the nursing profession in the state as they seek to streamline the nursing workforce, improve leadership and collaboration, and enhance nursing education.Key Messages that Structure IOM Recommendations.

Barriers to Advancement in New Mexico

The scope of practice barriers does not significantly face nurses in New Mexico. According to Intrator et al. (2015), nurse practitioners have full independent practice authority with state certification. Certified nurse practitioners practice independently and formulate decisions on the health care needs of individual patients, families, and communities, including the prescription of scheduled and controlled substances. The New Mexico state policy recognizes nurse practitioners as primary care providers, and they serve in acute care, chronic long term care, and end of life patient care. State laws and federal policies in the state, thus recognize nurses as independent primary care providers who are highly trained to provide a variety of clinical services in diverse settings. Other notable barriers facing nurses in the state are culture and outdated insurance reimbursement, models. The US continues to face significant disparities in health outcomes along the lines of race and ethnicity. As the state becomes more diverse, eliminating cultural barriers to effective nursing practice is crucial in advancing the profession.Key Messages that Structure IOM Recommendations.

Nursing advocates in the state are working to improve the above barriers. Their tireless efforts to modify the practice environments for nurses have contributed to recognizing nurse practitioners as independent clinicians. Nursing advocacy has also taken the lead role in reducing cultural disparities in the state. In New Mexico, cultural barriers to healthcare include compromised cultural relevance and distrust in healthcare systems and providers (Pfeifer, 2015).  In turn, this results in disparities in healthcare utilization in some states. Nursing advocacy centers on culturally responsive clinical services to increase the rates of utilization among populations. Further, advocacy intends to increase the number of the insured population in the state. Specifically, nurses are advocating forexpanded Medicaid eligibility to rural and underserved populations in New Mexico as a way to boost healthcare access and utilization.Key Messages that Structure IOM Recommendations.
