Benchmark: Evidence-Based Practice Proposal Project: Framework/Model for Change

Models of change are crucial in implementing evidence-based interventions in healthcare. There are multiple frameworks for guiding intervention implementation. One is the social-ecological model of health. The model takes a holistic perspective on health by focusing on multiple factors affecting health. The theory supports the World Health Organization’s constitution of 1947 that challenges healthcare providers to have a broad thinking of patient care. The social-ecological model views health as a product of interactions between individuals, the community, and physical, political, and social environments. The model has been applied in the implementation of various healthcare interventions, including mental health care (Agency for Toxic Substances and Disease Registry, 2023). In this paper, I demonstrate how to apply the social-ecological model of health in a community holistic approach for decreasing maternal mortality rates of opioid-addicted antepartum, intrapartum, and postpartum patients. The rest of the discussion focuses on each component of the change model and how each will be applied in implementing the proposed intervention. The social-ecological model is conceptualized to have four concentric circles representing multilevel factors that influence an individual’s health. The Centers for Disease Control and Prevention (CDC) identifies four components of the social-ecological model, including individual, relationship, community, and societal (See Appendix A). The first level (individual) involves a person’s biological and other personal demographics, such as health history, age, education, and income. The second level (relationship) includes an individual’s closest relations, including family members, partners, and friends, who affect a person’s behavior and influence their experiences. The third level (community) examines the contexts in which people form social relationships, such as neighborhoods, workplaces, and schools. It seeks to determine the specifics of these settings that influence health. The final level (societal) focuses on broad factors in society that promote or impair health. Examples of societal influences are social and cultural norms and social, educational, economic, and health policies that impact health (Agency for Toxic Substances and Disease Registry, 2023).

Opioid addiction in pregnant women is a significant problem in the United States. The cause of the problem is multifaceted and requires a holistic approach (Jalali et al., 2020). Opioid addiction in mothers is caused by individual, relationship, community, and society-based factors. It is associated with the biological and personal characteristics of a person. It may also stem from relating with family members, partners, or friends. Besides, it may result from the community /neighborhood where the individual lives. Opioid use disorder (OUD) may also be prevalent in communities or groups characterized by socioeconomic inequalities (Rizk et al., 2019).  Due to the broad factors related to opioid addiction, applying the social-ecological model of health to implement the proposed intervention is most appropriate. The change model aims to target interventions to each level of influence. At the individual level, the aim is to control opioid addiction and maintain maternal health. The strategies target mothers at the antepartum, intrapartum, and postpartum stages. The recommended treatment for opioid-addicted mothers is pharmacotherapy with naloxone, methadone, buprenorphine, and behavioral therapy, in addition to routine antenatal, intra-natal, and postnatal care. During prenatal, the community holistic approach recommends pregnant women be screened for substance use disorder, including OUD, as part of antenatal care. The healthcare providers should also make referrals for mental healthcare services the women may need.  At birth, there should be screening for neonatal exposure to drug addiction and access to relevant recovery services (Hoyland et al., 2022).

At the relationship level, the intervention calls for engaging the partners and family members of opioid-addicted mothers. The partners and the loved ones can play a significant role in the recovery and support of the individual. The social-ecological model shows that family and spouses can affect one’s health. For instance, opioid-addicted mothers require their partners or family members’ support to adhere to medications or take care of their newborns as they continue treatment (Rizk et al., 2019). At the community level of the model, the intervention proposes community partnerships that address opioid use and addiction control. Strategies that work at this level include public awareness about the risks of opioid use among pregnant women. The other strategy would be partnering with local community health clinics to adopt the practice of screening mothers for substance use disorders as part of their routine maternal care (Rizk et al., 2019).

Finally, at the societal level, the intervention proposes tailored interventions for opioid-addicted mothers. Acknowledging the healthcare disparities present in the U.S., the best approach to opioid addiction in marginalized communities or racial/ethnic minorities is community-based substance abuse treatment programs (Rizk et al., 2019). These community interventions help improve outcomes in socially and economically disadvantaged societies since they address social determinants of health.                The social-ecological model of health applies to the community holistic approach proposed for decreasing maternal deaths in antepartum, intrapartum, and postpartum patients.  The change framework examines health from a broad perspective. It postulates that treating an individual succeeds if we focus on the four components: individual, relationship, community, and societal. Using the social-ecological model, the intervention targets opioid-addicted mothers in antepartum, intrapartum, and postpartum. It also includes opioid addiction control strategies for families, communities, and society.
