The facility does recognize that it is essential to promote interprofessional collaboration through effective communication and teamwork among all the healthcare professionals in the facility. This helps to improve patient outcomes and in attaining transitions. The facility has set aside time for regular interdisciplinary meetings for the physicians, pharmacists, nurses, and therapists to meet and discuss various plans and patient cases. Here, knowledge and ideas are easily exchanged. In patient transitions, nurses play a very important role. The role of the nurses is to act as advocates for the patients to facilitate and coordinate the transition process. To achieve this, nurses work with other healthcare professionals to ensure appropriate discharge planning, smooth transfer of care, and effective communication of various patient needs (Nishiguchi et al., 2021). Nurses also educate patients and families regarding self-management strategies and post-transition care.

There are identifiable gaps in this process, especially regarding the general exchange of information and communication. Inaccurate or incomplete handoffs, insufficient time for interprofessional collaboration, and inadequate documentation can compromise the quality of care by hindering communication (Nishiguchi et al., 2021).
