It is a health program that utilizes remote patient monitoring to coordinate care beyond physical and geographical barriers (Grisot et al., 2019). Through a remote patient monitoring system, the nurse or any other healthcare provider can remotely receive patient health information, analyze it and advise on management care without physically assessing the patient. Who is your target population?
My target population for this program is diabetic patients. Globally, diabetes is one of the chronic non-communicable diseases that lead to the loss of millions of lives each year. The incidence of diabetes is about 7 in 1000 people especially in western populations (Khan et al., 2020). The increased incidence of diabetes relates to the increased cases of obesity. I also contribute to design by collecting information on patient preferences about the program.

As active participants of the health program, nurses can design the policies such as ethical guidelines that define the utilization of the healthcare program.
