In this experience, the patient is a thirty-eight-year-old male, who is on conservatorship, and who had been on our psychiatric unit for the last two years, as each time this patient was discharged to placement, the patient would be back on our unit within six hours, due to being unable to comply with the rules of each place of placement. To care for this patient, who at times is violent and impulsive, and has many TBIs, the patient has specific requests placed into the treatment plan as the patient can avoid many impulsive and violent episodes due to a reward system. The interesting thing about this situation is that the administration and about half of the nursing staff support the treatment plan, and half of the nursing staff do not support it. Hoffman et al. (2014) teach that with patients as the most important part of Shared Decision Making (SDM), their input is not taken as the most important in certain circumstances, and this is the case from what I have experienced, in psychiatric patient populations. Let it be noted that conversely, there is a wonderful number of nurses and providers that do a great job listening and attempting to incorporate the wishes of psychiatric patients. We experienced a three-month period where administration made it mandatory to follow the treatment plan, which added that this patient was able to drink coffee from the nurse’s station at any time day or night, and that the patient can watch fifteen minutes of music videos from a popular video service from the internet before going to bed. Interestingly, this falls into the social determinant of Neighborhood and Built Environment taught by Walden University (2023), as our facility is this patient’s home and has been for the last two years. Multiple times when the patient was not granted each of these activities that were written into the treatment plan, this patient will tell staff that they are not honoring the treatment plan that the Director of our unit has made mandatory after speaking with the patient, and then this patient will act out. Some staff sees that being mandatory means we must do this, and some see these activities of the patient manipulating the system and trying to get his way by being passive-aggressive and physically aggressive.

I selected the patient decision aid for depression within the scenario of treating mood disorders in older adults (which this patient has as a diagnosis) and is available through Universite’ Laval (2018) and can be used with this patient to figure out what to do in difficult circumstances and can also be used when helping patients make decisions in general psychiatric practice when I begin practice as a Psychiatric NP. The beauty of this tool is that it walks patients through the entire process where the guesswork is eliminated, going over who the patient is, why they need to make the decision, what priorities the patient needs to clarify, the different options the patient has to decide between, and after the decision is made, the entire process that the patient can go over if they need to refresh the information. In the scenario with our unit patient, educating the patient about his options was the best part of this process, as empowering him to decide was what bothered him the most. This even led to many times the patient letting nursing staff know that he did not need to participate in either activity of coffee or watching videos, but instead chose to go to bed with the scheduled patient lights out rules. In the future, there will be many decisions that need to be made when I will be practicing as a PMHNP. The same approach can be used, especially during difficult times, by going over the exact same steps and making the most informed decision that will lead to the best outcome personally as well as professionally. (2016) teach that especially with financially related to any activities that will advance practice professionally,  having a tool that helps you make hard decisions, is a great benefit. When I worked in the emergency department (ED), we saw a lot of patients that didn’t have insurance or means to pay for follow-up care and medications. Even with government assistance, it is difficult for some patients to afford their medications or follow-up rehab or appointments. One patient in particular came in with high blood pressure 200s over 100s, dizzy, lightheaded, massive headache, and on and off blurry vision. After treating the patient’s blood pressure and stabilizing them, I later found out that the patient hadn’t taken their blood pressure medication for over a month due to not being able to afford it after they raised the prices. He was barely able to afford the medication prior to the cost increase and keep food on his table, and roof over his head. Getting social work involved in his case allowed him to get resources and pharmacy discounts in order for him to actual save money on his medication. Without intervention from the social work resources, the patient wouldn’t have been able to afford the medication, continued to not take the medication, and most likely suffered a debilitating stroke. His outcome was improved by involving social work on his case in order to provide him the much-needed resources in order for him to afford his medication. Unfortunately, this case is an isolated one and many Americans can’t afford medications for their chronic medical conditions, which lead to worsening conditions, emergency situations, and eventually premature death. People without steady employment or income, that live in poverty, are mor likely to suffer from health issues and have limitations (Walden University Library, 2023). Most people suffering from this don’t know about the resources available to them to assist with medication costs, living expenses, and other support resources. Social determinants of health are defined as conditions in environments where people live, learn, work, and age that influence their health, functioning, and quality of life outcomes and risks (Walden University Library, 2023). Considering these factors on every patient will help determine the best way to help the patient have the best health outcome and get the resources they need in order to have proper compliance to treatment plans. Being considerate of these factors can put the situation on the right trajectory and lead to better patient outcomes and quality of life. The Ottawa Hospital (2023), provides many patient decision aids to help patients research their conditions and make the best educated decision on how to proceed with treatment. Their patient decision aid on high blood pressure: taking medication, meets seven out of seven criteria to be defined as a patient decision aid and meets seven out of eight criteria to lower the risk of making a biased decision (The Ottawa Hospital, 2023). Providing patient with aid can help them make a nonbiased decision on whether to take medication for multiple conditions and explains the risks and benefits of doing so. Within this aid, it displays options pertaining to taking medication or not, key points about the risks of not taking medications and taking them. This aid also provides a six-section exam that asks the patient questions about knowledge they have read and has a section on their feelings towards taking or not taking the medications. The aid also provides the patient with a summary of the facts, options, personal feelings, decision tree, and quiz results (Healthwise, 2023). This aid has value in educating the patient in making a nonbiased informed decision on how to pursue their disease treatment. It would be valuable to include in discharge teaching to my patients and would be useful for myself if I ever faced the decision of taking high blood pressure medication or not. Although, there is another side to this aid that isn’t addressed, and that is social determinants of health. No where in the patient aid doesn’t it address if the patient can’t afford the medications and doesn’t have the means to make the necessary lifestyle changes due to low income, poor access to healthcare, and low level of education to comprehend the risks and benefits of the medication. As mentioned in the patient case above, what if the patient doesn’t know about the resources available to them and can’t afford to pay the high prices to take the high blood pressure medications. These patient aids need to include the proper resources if you can’t afford the medication or have additional question pertaining to the treatment of the high blood pressure. Even though these patient aids are valuable to the general population, the patients that suffer from social determinants of health are at a disservice because there is in consideration for these factors.
