A 46-year-old, 230lb woman with a family history of breast cancer. She is up to date on yearly mammograms. She has a history of HTN. She complains of hot flushing, night sweats, and genitourinary symptoms. She had felt well until 1 month ago and she presented to her gynecologist for her annual gyn examination and to discuss her symptoms. She has a history of ASCUS about 5 years ago on her pap, other than that, Pap smears have been normal. Home medications are Norvasc 10mg qd and HCTZ 25mg qd. Her BP today is 150/90. She has regular monthly menstrual cycles. Her LMP was 1 month ago.

How might the underlying pathophysiology of these conditions affect the pharmacotherapeutics you might recommend to help address your patient’s health needs? For this Discussion, you will be assigned a patient case study and will consider how to address the patient’s current drug therapy plans. You will then suggest recommendations on how to revise these drug therapy plans to ensure effective, safe, and quality patient care for positive patient health outcomes.

The case study patient is a woman aged 46 years with a weight of 230lb and who hails from a family with a breast cancer history and HTN history. The patient has been in for a yearly mammogram, and last month, she visited her gynecologist for a GYN examination. Her initial complaints indicate that she experiences genitourinary, night sweats, and hot flushing symptoms. Also, despite having an ASCUS history for the last five years, her Pap smear is always normal. However, her blood pressure stands at 150/90, and she had regular monthly periods last month. The patient presents signs of perimenopause symptoms from the indicated patient history and examination. Hence, the main healthcare needs for the patients include hypertension management. As one of the hypertension managements, I would ensure that the patient discontinued using Norvasc but instead prescribe the use of Lisinopril 20 mg daily. The intervention is essential since it will help reduce the flushing symptoms (Sonn et al., 2019). However, there will be a need to evaluate blood pressure levels within two weeks to assess the new medication’s effectiveness. Once there is a realization that the blood pressure control is effective with lisinopril, HCTZ will be discontinued to reduce genitourinary symptoms.

Patient education will be crucial to ensure that patient carries out the recommended practice to improve the overall healthcare outcomes (Milman et al., 2018). Hence, there will be a need to engage a teach-back strategy to ensure that the patient understands well how to log in the blood pressure assessment to monitor and identify hypotension symptoms (Zabolypour et al., 2020). In conclusion, the patient will also need a comprehensive education to understand the effect of lisinopril, such as cloudy urine, blurred vision, and confusion, among other symptoms, and the need to seek medical attention once these symptoms become adverse.
