Choice of an Evidence-Based Topic for Clinical Inquiry

Clinical inquiry is aimed at cultivating the culture of best practice as opposed to common practice. Best practice is evidence-based practice or EBP (Melnyk & Fineout-Overholt, 2019). It is in this context that I identified and chose the topic of Management and Prevention of Gestational Diabetes Mellitus or GDM. This is a pertinent clinical topic because the worldwide prevalence of GDM is between 9.3% and 25.5% (Xu et al., 2017).Prevention and Management of Gestational Diabetes Mellitus (GDM).

Some of the potential BSN care practices that may require EBP change that I have identified include early screening and diagnosis for better management, timely referral for nutritional counseling, constant and regular blood glucose monitoring (including at home by the mother herself), health education on the complications of GDM to antenatal mothers, especially those who are overweight or obese, and follow-up screening in the postpartum period to determine remission.Prevention and Management of Gestational Diabetes Mellitus (GDM).

Some of the identified potential barriers to implementing EBP on the above BSN practices are (i) the presence of a language barrier (Spanish/ English) making it difficult to effectively communicate health promotion and prevention information, and (ii) resistance to change by mothers who are reluctant to let go of their lifestyles on especially diet and physical exercise.Prevention and Management of Gestational Diabetes Mellitus (GDM).
