How Competing Needs Impact the Development of Policy

     Globally, including in the U.S., healthcare policy aims to balance access, cost, and quality. However, these goals often conflict due to competing needs in healthcare delivery. As a result, the competing needs create tensions that compel lawmakers involved in healthcare reforms to make nuanced compromises across interdependent facets of healthcare delivery in order to gradually improve system outcomes (Kelly & Porr, 2018). Some of the competing needs in healthcare delivery, such as healthcare workforce shortages, limited financial resources, and rising patient demands, create difficult tradeoffs in healthcare policymaking. While the workforce demands better pay and working conditions, these needs must be balanced against the high costs of healthcare and limited budgets. As such, policies aimed at improving healthcare access and quality of care to patients must compete with policies focusing on healthcare investments and reforms. According to Kruk et al. (2018), effective healthcare policies should prioritize the interrelated competing needs through compromise and evidence-based analysis of healthcare system incentives, costs, and expected benefits.      The national healthcare issue plaguing the U.S. healthcare system is the nursing shortage. The nursing shortage is impacted by the competing needs of nurses, healthcare facilities, and patients. According to Shah et al. (2021), nurses experience high stress and burnout risks from understaffing, long shifts, and increased patient loads. This often leads to high nurse turnover since healthcare facilities are unable to provide better working conditions, scheduling, and pay that would enable them to attract and retain nurses. This is because the healthcare facilities face financial pressures from lower reimbursements and high labor costs, limiting their flexibility (Shrank et al., 2021). At the same time, patients have increased medical needs and demands for accessible, affordable quality care that require more nurses per patient. Balancing these nurse workforce needs, institutional resource constraints, and rising patient demands pose a major challenge to policymakers aiming to resolve nurse shortages. Policies enacted to address these competing needs should focus on improving nurses’ working conditions and staffing ratios. The policies should also aim to control healthcare costs through increased reimbursements and nurse training and retention incentives.
