My experiences in this course have finally put all the pieces together. I am glad that this course focused on the diagnosis as well as the medications, as making those connections and having a brief overview of diagnostic criteria was helpful. I now see why some primary care providers may not be equipped enough to adequately care for patients with mental health needs. • What did you expect from participating in this course and were your expectations met? Why or why not?

I expected to learn about medication classes and who to prescribe them to. Both of those aspects were met in this course with great detail. The weekly quizzes added to the course as it helps to see what a patient encounter may be like and how much information we need to know to be able to properly help the patient. In working as a nurse currently I have had patients on SSRI’s such as Paxil, and I had found myself wondering how or why that patient was started on it.

• Did you have some preconceived biases that affected your experience as you worked through the course?

The only preconceived bias I had prior to this term, was just my bias towards how well Seroquel works. After learning in this course, I see that in the ICU we tend to use it off-label to use as sedation instead of an actual FDA approved use. I will say that I have seen how effective Seroquel is as sedating even at lower doses and moving forward if I prescribe it to patients I will be sure to make them aware that it usually does cause sedation. The “ah ha” moment I had during this course was my understanding of bipolar disorder and mood stabilizers. I had come across many patients in my time as a bedside nurse that were on what I assumed were medications to prevent seizures, but now looking back, I bet a lot of the patients were actually bipolar. It was confusing to me why a patient would be on an anticonvulsant and not have a seizure history.  This was a big moment for me, mostly because I am trying to transition to the provider role and understand why medications are prescribed instead of just knowing the drug class and its usual uses.

• How have you, your practice, or your ideas about prescribing psychopharmacological interventions changed since participating in this course?

My ideas about prescribing are now that it is truly an art. There isn’t one drug that will work for everyone. I never understood why there were so many different SSRI’s. And never understood how adding different medication classes could help a patient with depression. My biggest take away from the class is that I will be educating all patients I see on how the medications they start are supposed to work, how fast, and what the plan will be if it doesn’t work at all. I think a lot of primary care providers do not have the ability to follow up frequently with patients and help find the right medication for them, but that is what I am going to be committed to doing. • What was the most significant thing that you have learned from participating in this course?

I learned that the patient should be involved in the option of medication management. I learned how it is really a joint effort to prescribe a medication, and to make patients aware of potential side effects, and what the next steps would be if the given treatment doesn’t work. I have heard many people tell me how they started an SSRI and it wasn’t working so they stopped taking the drug and never went back to see their provider. I think that this could be avoided if the patient is made aware up front that the medication may take time to work, and may not work at all, and to have a follow up visit and plan in place for this situation.
